Standard application form

The Standard Application form (Form1) and the related Applicant’s checklist on the supporting documentation (Form2) to be used are available from the Department of Health website in section: Policy A-Z>C>Clinical Trials Involving Medicinal Products.
The application form must be signed by the Chief Investigator. The application form together with all the necessary supporting documentation should be provided.
