The local ethics committee should be referred to for the preferred format.
Standard application form
There is a standard form for clinical investigations of MD and other health-related research studies which are not clinical trials of medicinal products for human use as defined in SI 190/2004, which is used by a number of ECs.
Some local ethics committees may request a different format.
